Send your prescription and quote to your insurance company to request coverage of Jiobit and the monthly subscription.
Each state program has different requirements. Details on your local state program can be found at the Medicaid website. You may need a diagnosis and prescription as detailed out above.
You'll need to apply to be approved for the Medicaid Waiver. You should be assigned a caseworker to help. Some offices do require a W-9 form.
Once you're approved for the Medicaid Waiver, have your caseworker contact us so that we can become an approved vendor for your state office. We will complete the paperwork ASAP.
